en/index: try to simplify the markup and make it more accessible

- Replace <span style="..."> with
    <code> for commands,
    <kbd> for keyboard shortcut,
    <samp> for program output,
    <i> for menu options.
- Make <a> a child of <strong> in the Translate option & remove <span>
  (the color of <a> overrides the color of <strong>).
- Replace h4 with h6.
- Remove unnecessary <br />.
