The 'State' select field does not update when the 'Country' select
field is changed. I guess CORS must be enabled at crm.fsf.org for this
to work.

- LibreJS accepts the JS.
- Changing the 'Country' doesn't change the 'State' select field.

    The following error occurs:

    Cross-Origin Request Blocked: The Same Origin Policy disallows
    reading the remote resource at
    https://crm.fsf.org/civicrm/ajax/jqState?_id=country-Primary&_name=country-Primary&_value=1241. This
    can be fixed by moving the resource to the same domain or enabling
    CORS.

    I guess I need to contact sysadmin for this.

Removed JS includes from footer.html. From now on JS includes will put in a seperate file residing at /server/2015/

This change was done because each page will have different JS
requirement.

All pages, from now on:

- Will have to include an additional /server/2015/close.html file that
closes the HTML document.

- There will be three SSI includes at the end of each HTML document.
