CRM-16173 - Cxn API - Add "get". Fix GUID fields and API metadata.

APIv3 overloads fields like "cxn_id" as aliases for numeric "id". Therefore,
it's problematic to use "cxn_id" and "app_id" as GUID fields.  This commit
renames them to "cxn_guid" and "app_guid".
